Construction Hazard Recognition – Trainer Guide

Program Overview

In this experience, users move through the construction site and identify ten hazards. Once a hazard has been identified, they will be asked to identify different possible mitigations for the hazard using the Hierarchy of Controls developed by OSHA. Hazard appearance is randomly determined, so each experience will be different.

Core Learning Objectives

The goal of this module is to instruct users in identifying and correctly rectifying potential hazards using the best methods possible:

  • Hazard Identification — identify, assess and prioritize both existing and potential hazards

  • Hazard Prevention — identify, evaluate and control risks that could harm people, property, or the environment. This involves regular inspections, risk assessments, and implementing control measures such as elimination, substitution, engineering controls, administrative controls, and personal protective equipment.

Tools

  • Laser Pointer — To interact with menus and objects in this experience, aim your laser pointer at the item you wish to interact with and pull the trigger on your controller to select it.

How to Get 100%

Identify the ten hazards that are present within the construction site. Hazards that still need identification will have a Blue Arrow marker placed above them.

 

The Assessment Status scoreboard can be used to check on progress throughout the scenario. The scoreboard will update to show which hazards have been interacted with and which hazards remain.

 

  1. Once a hazard is found, select it with the Laser Pointer and identify it.
  2. Select possible mitigations for the hazard. This may require selecting more than one option depending on the hazard. See the Hierarchy of Controls developed by OSHA for hazard mitigation recommendations:

  1. Select Done to close out the menu.
    1. If the wrong hazard was selected, use the Reselect Hazard button in the bottom left corner to go back and select a different hazard to report.
  2. After making a selection, a Green Check Mark, Yellow Check Mark, or Red X marker will appear indicating whether or not the correct selection was made. For any incorrect selection(s), the correct selections will be noted.
  3. Continue until all hazards within the environment have been assessed.

Anywhere

  1. Saw Cutting Lumber or Metal 

    1. Cord is Obstructing Access; Worker Still Proceeding

    2. Sparks Traveling at Fuel Source

    3. No PPE

  2. No PPE / Missing PPE Appropriate to Task

  3. “Danger” / “Do Not Enter” Signs Missing or Damaged

  4. Moving Equipment

    1. Workers Exposed to Heavy Equipment with No Safety Measures (barricades, warning signs, flagman)

    2. Defective Crane Latches

Building Area

  1. Trash Piles / Debris

  2. Workers

    1. No Masks (for dust)

    2. Not Tied Off

    3. Not Wearing Harness

    4. Scaffold has Red Safety Tag / Missing Safety Tag

    5. Not Wearing PPE

    6. Exposed Sides / Rear

    7. Material Unsecured on Scaffold, Near / Hanging from Edge

  3. Breaker Panel — Front Panel Removed; Live Wires Present

  4. Worker on Ladder 

    1. Only two points of contact (leaning out to work) 

    2. Ladder on Debris Pile or other Unstable Surface

  5. Welding near Flammable Supplies (paint, fuel, etc.)

  7. Crane too Close; not Properly Shored 

  8. No Trench Wall Supports

Material Storage Area

  1. Unsecured Supplies (oxygen tanks, fuel, etc.)

  2. Spilled Chemicals

  3. Learning Materials

  4. Debris Piles; Large Materials (pallets, lumber, etc.) 

  5. Welding / Cutting Area throwing Sparks into Ignition Source (paper, wood debris, fuel) 

  6. Worker Resting under a Shaded Area where Gases are Entrapped; Lack of Signs / Barricades in the Area

  7. Permit Box Displayed within the Immediate Work Area to Showcase Missing Permit

Roadway Area

  1. Forklift with No Driver, but Carrying Load

  2. Crane Operator Operating without a Rigger or with Faulty Latches
