Hazard Recognition Warehouse – Trainer Guide

Program Overview

In this experience, users explore a warehouse workplace where safety isn’t a top priority. They must use and apply their knowledge of General Industry Standards to locate dangerous health and safety risks.

Core Learning Objectives

  • Hazard Identification — identify, assess and prioritize both existing and potential hazards

Tools

  • Laser Pointer — to interact with menus in this experience, aim the laser pointer and pull the trigger on the controller to select an item.

  • Virtual Hands — to activate the green buttons within the environment, physically press them using virtual hands.

How to Get 100%

  1. Grab and apply the proper PPE from the PPE Locker (located in the preparation room).

    1. Tip: Apply the PPE by physically bringing the equipment close to body using virtual hands.

    2. Possible Defects:

      • Damaged Glasses 

      • Damaged Hardhat 

      • Missing Hard Hat

      • Worn or Damaged Gloves 

      • Worn or Damaged Earmuffs

  2. Open "Warehouse Entrance" Door

  3. Locate and report hazards.

    1. Tip: Use the Laser Pointer to highlight and select interactable items within the scene.

  4. After selecting an item, determine if the item is a hazard or not. Each item will have different selection options that pertain to hazards in relation to that object.

  5. When finished, press the Complete Inspection button in the PPE room to enter Review Mode.

Possible Hazards

  1. MSDS — Missing or Mislabeled

  2. Ladders

    • Missing Rungs / Inadequately Repaired / Bent Leg

    • Blocking Pathway

    • Wrong Angle for Work

    • Inadequate Clearance at Height

  3. Containers

    • Damage or Corrosion

    • Improper Type / Improperly Stored / Dangerously Grouped

    • Incomplete / Torn / Overwritten / Missing / Incorrect Label

  4. Gas Cylinder Pack

    • Corrosion

    • Horizontally Stored

    • No Chain

    • Dangerously Grouped Flammable in Pack

  5. Gas Cylinder Cage

    • Missing or Torn Sign

    • Cage Door is Open

    • No Chain

    • Mixed Materials

    • Mislabeled

    • Corrosion

  6. Workers / Colleagues

    • Missing PPE — Gloves / Hard Hat / Glasses / Chemical Apron / Chemical Gloves / Face Shield / Chemical Goggles

    • Improper Use of Ladder or Scaffolding

  7. Stairs

    • Railings Too Low

    • Missing Toeboard or Railing

  8. Scaffolding

    • Missing — Outriggers / Pins / Railings / Toeboard / Walkways

    • Damaged Frame

    • Wheels Unlocked

    • Blocking Pathway or Exit

  9. Flammable Closet — Improper Storage

  10. Palettes

    • Blocking Pathway or Exit

    • Carelessly Stored

  11. Forklifts

    • Employee on Forks

    • Employee on Back of Deck

    • Driver Inspection under Load / Dangerous Inspection Conditions

    • Barrel Resting on Forks

    • Visible Leak

    • No Operator — Forks Down

    • No Operator — with Barrel Load

    • Driver’s Load is Obscuring Vision Line
